Criteria
- • Financial need
- • United States Citizen or have permanent resident status
- • Current dental hygiene student in good academic standing
- • Evidence of Community Service
Include with Application
- • Letter of request for consideration including the need for financial support
- • Brief description of community service activity in 500 words or less
- • SAR or GAP Financial aid report from FAFSA submission
- • Two Letters of Recommendation:
- - One must be from the Program Director
- - One must be from a NDHA member including your membership status
- • Reapplication in subsequent years, if eligible
